The AI Agent Accountability Gap: 74% of Knowledge Workers Use AI, But Nobody Owns It
Here's the question that nobody in your organization can answer: which AI agents accessed which data, when, and on whose authority?
Today — March 26, 2026 — Fortune published a study with Accenture and Wharton that puts the scale of the problem in sharp relief. Nearly 74% of knowledge workers are now using AI. Not in a sanctioned, governed way. In the shadow AI way: bring-your-own-AI-tools, deployed by business units, without IT approval, without security review, without a conversation about what happens when they go wrong.
Simultaneously, at RSAC 2026 in San Francisco this week, CSA and Aembit released survey data from 228 IT and security professionals that makes the organizational reality even starker. Sixty-eight percent of organizations cannot clearly distinguish AI agent activity from human activity in their own systems. Eighty-four percent doubted they could pass a compliance audit focused on AI agent behavior and access controls.
Seventy-four percent of your knowledge workers are using AI. Sixty-eight percent of organizations can't tell whether the actions in their systems were taken by a human or an AI agent. Nobody owns this.
That's the accountability gap. And it's not a technology problem. It's an organizational design problem.
The Numbers Behind the Accountability Gap
74% of Knowledge Workers Using AI — Most of Them Ungoverned
The Accenture and Wharton Global Products report, published in Fortune today, is the anchor for this section. James Crowley, co-author: "Intelligence may be scalable, but accountability is not." That sentence is the thesis.
The scope: 120 million workers across 18 industries. More than 50% of working hours across the American economy are in play — subject to reshaping by AI agents. Banking and capital markets: more than 45% of hours impacted by digital agents alone. By 2028, roughly one in three enterprise applications is expected to embed agentic capabilities.
The governance response to this deployment has not kept pace. The majority of knowledge workers using AI are doing so through unsanctioned tools. Business units are adopting AI faster than IT can evaluate it. Security teams don't have visibility into what agents are running, what they're accessing, or what decisions they're making.

The CSA/Aembit survey, published March 24 at RSAC 2026 and covered by Help Net Security on March 26, is the operational counterpart to the Fortune data.
Eighty-five percent of organizations have AI agents functioning in production environments. That number is consistent with the Accenture+Wharton finding — agents are deployed, they're operational, they're making decisions.
But 68% cannot clearly distinguish between human-initiated and AI-agent-initiated activity in their own systems. That's not a minor gap. That's a fundamental identity problem. If you can't distinguish between what a human did and what an agent did, you can't attribute actions, enforce accountability, or investigate incidents.
The compounding data: 73% expect AI agents to become vital to their organizations within the next year. The deployment rate is accelerating. The governance gap is widening with it.

The Okta and Accenture joint webinar, from January 23, 2026, gave us the governance effectiveness contrast: 91% of organizations are already using AI agents. Only 10% feel they have an effective AI agent governance strategy.
Ten percent. That's the number that should keep every CISO awake tonight. Almost every enterprise is running AI agents. Almost none of them have governance that works.
Greg Callegari from Accenture put the diagnosis plainly on that webinar: "Agents are acting like employees, they perform tasks humans would do. So, the way to secure them is by managing them as identities." The organizations that haven't accepted that premise are the ones running at 10% governance effectiveness.
Why Nobody Owns AI Agent Accountability
The accountability gap isn't accidental. It's structural. The CSA data makes the ownership fragmentation visible in numbers: responsibility for AI agent governance is scattered across four constituencies, none of which has it as a primary mandate.
Security leads: 28%. Development and engineering: 21%. IT: 19%. IAM teams: 9%.
Nine percent. The team most qualified to manage non-human identity governance — IAM — is leading AI agent governance at just 9% of organizations. Everyone else is improvising.
The Wharton Accountable AI Lab's Kevin Werbach described the organizational dynamic that creates this vacuum: "My business program manager is making agents and throwing them out there." The traditional IT release review process can't keep pace with the speed at which agents are being built inside organizations. By the time a governance review is scheduled, the agent has been running in production for six weeks.
The result: accountability that's everyone's responsibility and nobody's job. Security thinks IAM owns it. IAM thinks security owns it. Development built some of them. Business units built others. Nobody called a meeting to decide who was responsible for what happened when the agent did something unexpected.
The Authentication Gap
The technical dimension of the accountability problem is equally stark.
From the Strata Identity and CSA research, published February 5, 2026: 44% of organizations use static API keys to authenticate AI agents. Forty-three percent use username and password combinations. These aren't legacy systems. These are production AI agents, running autonomously, using the same credential model as a human employee's login.
Static API keys don't expire automatically. Username and password credentials aren't tied to a specific agent's identity. When an agent is compromised, those credentials remain active until someone manually revokes them. Eric Olden, CEO of Strata Identity: "Static credentials, manual provisioning, and siloed policies can't keep pace with the speed and autonomy of agentic systems."
Thirty-one percent of organizations allow AI agents to operate under human user identities. That means the agent is using the same credentials as a specific employee — not a service identity, not an agent identity, but an actual human's login. When something goes wrong, the audit log shows a human name. The human was in a meeting at the time.
The Security Implications — When Agents Go Rogue
The accountability gap isn't just a compliance problem. It's a security attack surface.
The CSA/Aembit data: 74% of organizations report that AI agents often receive more access than necessary for their specific task. Seventy-nine percent say agents create new access pathways that are difficult to monitor. These aren't edge cases. These are the operational norm.
An AI agent with overly broad permissions, operating autonomously across multiple systems, is the definition of expanded attack surface. It's not a malicious insider. It's not an external attacker. It's an autonomous system doing exactly what it was designed to do — with access that nobody scoped properly.
The prompt manipulation risk compounds this. Eighty-one percent of organizations agree: prompt manipulation could cause an AI agent to reveal sensitive credentials or tokens. The agent uses real credentials through a real access path. No malware was planted. No exploit code was used. The agent was manipulated into acting against the organization's interests through the same interface it's supposed to use.
Arize published "100 AI Agents Per Employee" on March 21, 2026. Jensen Huang's prediction: roughly 100 AI agents per employee at major enterprises within years. McKinsey's current state estimate: 25,000 agents working alongside 60,000 humans at a typical large enterprise. Scale that math across your organization. Each agent has some level of system access. Most of them have more than they need. Most of them aren't monitored. Most of them operate under credentials nobody scoped.
That's not a security posture. That's an attack surface waiting for a trigger.
The Governance Fix — Treat AI Agents Like Employees
Greg Callegari's framework from the Okta webinar is the most actionable governance principle I've seen articulated for AI agents: "Agents need their own identity. Once you accept that, everything else flows — access control, governance, auditing and compliance."
Treat AI agents like employees. Not metaphorically. Operationally.
No organization would hire an employee, hand them admin credentials across every system, and hope for the best. They'd define a role. Enforce least privilege. Monitor activity. Establish who is accountable when something goes wrong.
AI agents need exactly the same treatment. Here's what that looks like in practice.
Step 1: Give Every Agent Its Own Formal Identity
Not a shared service account. Not a human employee's login. A distinct, attributable identity — with its own credentials, its own access scope, and its own ownership chain.
This is the foundation. Every other governance step depends on it. Without agent-level identity, you cannot attribute actions, enforce access controls, or conduct meaningful incident investigations.
Step 2: Scope Access Like You Scope Access for a New Employee
Define exactly what each agent needs access to, for exactly which tasks. Agents should operate on least-privilege access — exactly what their function requires, nothing more.
The CSA data — 74% of agents receive more access than necessary — reflects the current default: agents get broad access because it's easier to configure. That's the same reasoning that created the excessive permissions problem for human employees in the 1990s. We solved it then with role-based access control. We need to solve it now for AI agents.
Step 3: Define Agent Lifecycle Management
Agents have start dates. Agents have review periods. Agents have end dates.
When an AI agent's assigned task is complete, its access should be reviewed and, if appropriate, revoked. When an agent is decommissioned, its identity should be formally retired — the same as when an employee leaves.
Most organizations have no agent lifecycle process. Most agents run until something breaks or nobody remembers they exist. The agents that continue running indefinitely with active credentials are the ones that become security incidents.
Step 4: Continuous Monitoring and Attribution
Agent behavior should be logged, attributed to a specific agent identity, and monitored continuously — not reviewed after an incident, but tracked in real time as a standard operational practice.
This is where the 68% — organizations that can't distinguish agent activity from human activity — needs to start. You cannot monitor what you cannot distinguish. Build the attribution infrastructure first.
Step 5: Assign One Owner With Full Accountability
The ownership fragmentation — security 28%, dev/eng 21%, IT 19%, IAM 9% — is the structural reason governance fails. Governance without a named owner is governance without enforcement.
One team should own AI agent governance. For most organizations, that's IAM — the team already responsible for non-human identity management. For others, it might be security. The specific team matters less than the principle: one owner, with explicit accountability, with authority to enforce policies.
The Accountability Self-Assessment — 8 Questions Every Executive Should Be Able to Answer
Use these eight questions to assess where your organization stands. A "no" or "I don't know" on most of these is a diagnostic, not a verdict.
1. What percentage of the AI agents running in your production environment went through formal IT and security review before deployment?
If the answer is "most of them" or "I don't know," you have a governance problem. The Accenture+Wharton data suggests it's the latter.
2. Can you distinguish, in real time, which actions in your systems were taken by a human versus an AI agent?
If no, you cannot attribute actions, investigate incidents, or conduct compliance audits. The 68% who can't distinguish is your peer group.
3. Who owns AI agent governance at your organization?
If the answer involves the word "we" — as in "we all do" — nobody owns it. Ownership requires a single name.
4. What percentage of your AI agents operate under their own formal identity versus a shared service account or human employee's credentials?
If most agents use shared credentials, you cannot attribute actions to specific agents. You cannot revoke a shared credential without affecting every agent that uses it.
5. Could you pass a compliance audit focused specifically on AI agent behavior and access controls?
The CSA data: 84% of organizations doubted they could. If your answer is anything other than "yes," the gap is your exposure.
6. Do your AI agents have defined access scopes — or do they have more access than their specific tasks require?
If agents have blanket access to systems rather than task-specific access, you have the 74% problem — elevated permissions that create unnecessary attack surface.
7. What happens to an AI agent's access when its assigned task is complete?
If the answer is "it keeps running," you have agents with active credentials and no defined end state. That's the lifecycle problem.
8. Do you have an incident response plan specifically for a compromised AI agent?
If the answer is "we'd handle it like any other security incident," you don't have an AI-specific plan. A compromised agent operates differently than compromised human credentials — your response should reflect that.
Bottom Line
The accountability gap is not a technology gap. It's an organizational design gap.
